Additional documents may be required based on the commodity type, destination, parties involved and desired services when shipping internationally.

Certificate of Origin or Manufacturer’s Affidavit

Some countries require a signed Certificate of Origin for all commodities while others only require one for certain commodities.

In many cases, a statement of origin printed on company letterhead will suffice. When a signed certificate is required, you can usually obtain a Certificate of Origin through organizations such as your local Chamber of Commerce.

A Certificate of Origin includes the following required information:

  • Undersigned – name of the owner or agent
  • Shipper’s name and address
  • DHL as the carrier
  • Date of pick-up
  • Consignee’s name
  • Marks and numbers on each package
  • Number of boxes or cases
  • Gross weight of each package
  • Net pounds
  • Description of item
  • State of Certificate of Origin
  • Country of Certificate of Origin
  • Signature by official stating document is factual
  • Date of signature
  • Signature of owner or agent
